What this is
New York's Department of Health retired its standalone Adult Care Facility Directory on January 15, 2022 and moved the data into general health-facility files, mixed with hospitals and clinics. This site rebuilds the directory from those public files: each adult home and enriched housing program, its operating certificate, operator and certified beds, joined on the certificate number to the State's quarterly report of capacity and residents. Facilities that were in the 2021 directory and are no longer in the State file are kept and marked as such. This site keeps the companies — names that carry Inc, LLC, Corp and the like — and makes them searchable by name, type and place.

What it is not
Not New York State Department of Health, and not a certificate. Individuals and names without a company word are left out. Rebuilt from four State files; nothing is added from any other source. “Beds not occupied” is certified capacity minus the residents the facility reported for that quarter — not a promise of a vacancy today. Phone numbers and resident statistics by age or sex are not shown.
